Can I continue seeing my other doctors?

Yes. Palliative care works alongside your primary care providers and specialists; we do not replace them. Our team collaborates with your existing physicians to enhance your overall care plan.

Does insurance cover palliative care?

What is geriatric-focused primary care?

Geriatric primary care is designed specifically for older adults. It focuses on managing multiple chronic conditions, medication safety, fall prevention, cognitive health, and maintaining independence.

What is Chronic Care Management?

Chronic care management (CCM) is a Medicare covered service for patients with two or more ongoing health conditions. CCM provides monthly care coordination, medication oversight, and direct access to your care team between visits- helping prevent complications and prevent hospitalizations.

Who qualifies for home visits?

Home visits are available for patients who have difficulty leaving home due to mobility limitations, advanced illness, cognitive impairments, or medical complexities. If leaving home requires significant effort or assistance, you may qualify.
