Palliative Care
Palliative Care is specialized medical care focused on improving quality of life fore patients living with serious or complex medical illness. It can be provided alongside curative or life-prolonging treatments at any stage of illness.
Our palliative care services include:
Symptom management (pain, shortness of breath, nausea, fatigue, anxiety)
Advanced care planning and goals-of-care conversations
Support with complex medical decision making
Coordination with primary care, specialists, and home health or hospice when needed
Emotional and caregiver support
Telehealth
Telehealth appointments provide convenient access to care for patients who are unable to come into the office or who prefer virtual visits for certain needs.
Telehealth is appropriate for:
Follow up visits
Medication Management
Chronic disease check-ins
Care coordination or care planning discussions
Select acute concerns
Telehealth services are delivered through a secure platform and allow us to maintain continuity of care while reducing travel and wait times.
