Primary Care

Our primary care services focus on comprehensive, relationship-based medicine for adults. We manage both acute and chronic conditions with an emphasis on prevention, and whole-person care.

Services Include:

  • Chronic Disease Management (hypertension, diabetes, arthritis, etc.)

  • Acute illness evaluation and treatment

  • Medication Management and reconciliation

  • Preventative care and wellness visits

  • Care coordination with specialists and hospitals

  • Health screenings and routine monitoring

Home Visits

For patients who have difficulty traveling, we offer home-based primary care and palliative care visits.

Home visit services include:

  • Comprehensive medical assessments

  • Chronic condition management

  • Medication review and safety checks

  • Fall risk and functional assessments

  • Caregiver education and support

  • Coordination with home health, therapy. and community resources

Home visits allow us to deliver high-quality care while gaining a deeper understanding of each patient’s daily environment and needs.

Palliative Care

Palliative Care is specialized medical care focused on improving quality of life fore patients living with serious or complex medical illness. It can be provided alongside curative or life-prolonging treatments at any stage of illness.

Our palliative care services include:

  • Symptom management (pain, shortness of breath, nausea, fatigue, anxiety)

  • Advanced care planning and goals-of-care conversations

  • Support with complex medical decision making

  • Coordination with primary care, specialists, and home health or hospice when needed

  • Emotional and caregiver support 

Telehealth

Telehealth appointments provide convenient access to care for patients who are unable to come into the office or who prefer virtual visits for certain needs.

Telehealth is appropriate for:

  • Follow up visits

  • Medication Management

  • Chronic disease check-ins

  • Care coordination or care planning discussions

  • Select acute concerns

Telehealth services are delivered through a secure platform and allow us to maintain continuity of care while reducing travel and wait times.
