Our Story

Elite Primary & Palliative Care was founded with a deep desire to serve the community where it all began—Brazoria County. Born and raised in this area, the practice’s founder recognized the unique needs of the community and envisioned a different kind of healthcare experience—one rooted in compassion, presence, and genuine connection.

From the beginning, Elite was created to provide patient-centered care that honors the whole person—mind, body, and spirit. Years of experience across primary care, home health, hospice, and hospital settings revealed a recurring gap in healthcare: patients often felt rushed, unheard, or left to navigate complex medical decisions alone. Elite was formed to restore what was missing—time, trust, and thoughtful care.

What began as a primary care practice quickly evolved as patients’ needs became more apparent. Many required not only medical management, but also deeper support through chronic illness, life transitions, and serious health challenges. In response, Elite expanded its services to include palliative care, allowing the team to walk alongside patients and families with greater intention, clarity, and compassion—at any stage of illness.

Today, Elite Primary & Palliative Care provides comprehensive primary and palliative services in the clinic, in patients’ homes, and through telehealth. Led by clinicians who remain closely connected to patient care, the practice is guided by real-world experience and a commitment to excellence.

At its heart, Elite exists to serve. Every decision, care plan, and patient interaction is shaped by a calling to honor dignity, foster peace, and provide care that feels personal—because healthcare is most meaningful when it reflects the values of the community it serves.

Joe Vasquez APRN, AGNP-C

Chief Clinical Office | Lead Nurse Practitioner | Founder

Joe Vasquez APRN, AGNP-C, serves as the Chief Clinical Officer and lead nurse practitioner at Elite Primary & Palliative Care, caring directly for patients. Joe brings extensive experience in adult-gerontological primary care, hospital medicine, and geriatrics. His clinical background allows him to care for patients with complex medical needs while maintaining strong focus on excellence, safety, continuity, and quality outcomes.

Joe is known for his thoughtful, stead approach to care. He believes trust is built through presence and listening. He takes time with patients and families, explains care plans clearly, and focuses on what matters most to each individual. His leadership helps set the clinical standards across the practice, ensuring care plans are best practice, evidence based and aligned with patient’s goals.

Outside of work, Joe find his greatest joy in faith, family, and community. He and Natalie love spending time with their families and serving in their local church. A watch enthusiast, Joe appreciates the craftsmanship behind every timepiece, seeing each one as a reminder to cherish the moments God gives.

Natalie Roman-Vasquez APRN, FNP-C

Chief Executive Officer | Founder

Natalie Roman-Vasquez APRN, FNP-C, is the chief executive officer and co-founder of Elite Primary & Palliative Care. With a background spanning primary care, home health and hospice, and healthcare leadership, Natalie brings a unique blend of clinical insight and operational expertise to the practice. Natalie believes healthcare is deeply personal and that every patient deserves to be treated with dignity, respect, and genuine compassion. Her heart for service and commitment to excellence shine through her every interaction from fostering community partnerships, educating referral sources, or treating patients.

As CEO, she guides the strategic and operational direction of the practice while maintaining a strong clinical foundation.

Outside of work, she treasures time with Joe and her children and enjoys being active in their local church and community.
